Veterinary Referral Form
If your animal is undergoing or has had recent veterinary treatment, or has a known condition that you are hoping physiotherapy can help with, please complete the following form (to be signed by your vet) before booking an appointment.
Previously, veterinary consent was required before any animal could receive physiotherapy. As of November 2020, the RCVS released new guidance within Chapter 19 in the Code of Professional Conduct which states that musculoskeletal therapists would not have to seek veterinary referral for maintenance care. Therefore, veterinary referral will be only required from patients currently under the care of a veterinarian and not for maintenance treatments.
